Rwenzori Center for Research and Advocacy has received funding from Baylor-Uganda to strengthen the “Bi-directional clinical, community and OVC referrals, linkages and service delivery implementation strategy”. The implementation theme will be “Utilizing community structures to reach the 95-95-95 targets”.

Activities will include:

1st 95 – Identification of People Living with HIV/AIDS (PLHIV) and Tuberculosis (TB) and index client contact tracing.

2nd 95 – Linkage to ART services through the differentiated service delivery models.

3rd 95 – Supporting adherence to and retention on antiretroviral therapy (ART)

  1. Linkages and Referral Supervisor- 1 position
Job Title: Linkages and Referral Supervisor
Job Location: Kasese
Job Summary:

The linkages and referral supervisor will mainly coordinate the implementation of community-facility framework to ensure achievement of the 95-95-95 targets. She/he will promote effective coordination with clinical, local governments, community structures, police, other CSOs, cultural & religious institutions among others to support the effective implementation of referral and linkage activities within the project framework.

The LRS shall be responsible for supervising and coordinating activities of Community Health Workers (CHWs) to scale up bi-directional referrals and linkage between the community and supported Health Facilities and other institutions; He/she shall work jointly with the DHE/Sub-county Health Educators/Health Assistants to identify the most suitably qualified community health workers to engage in Community-Facility Framework Activities; Ensure that all lost clients and clients who miss clinic appointments are followed up and returned to care in real time; Scale up Community Models of Differentiated Service Delivery (DSDM) by mobilizing and organizing clients into CCLAD groups, and coordinating CCLAD and CDDP outreaches; Jointly work with HIV Clinics to generated lists of clients for follow up and TB/HIV contact tracing/reverse-contact tracing. Coordinate and manage referral and linkages between RCRA and other institutions to achieve Accelerated HIV Epidemic Control in Kasese District.

 

Required Qualification, Experience and Competencies:

Essential

a.       A Diploma or higher in humanities, preferably in Social Sciences; public health studies, Development Studies.

b.      She/he should have demonstrated ability and experience to manage community  based structures

c.       Able to develop periodic work plans and timely reports

d.      She/he should have demonstrated skills to work in project teams.

e.       She/he should have computer skills including MS Word, Excel, PowerPoint, data entry etc.…

f.       Demonstrable good communication skills in both English and Local Language

g.      Skills and experience in coordination and networks building

Desirable:

h.      Driving permit (class A- Motorcycles

i.        Experience in building a strong referral and linkages system for HIV care and treatment services

j.        Additional training in M& E shall be an added advantage.

Key Duties

a) Utilize and update project service mapping data to generate complete and functional Referral services directory

b) Strengthen documentation of referrals and linkages for different services within the integrated project to ensure a continuum of care and response.

e)Take lead in the establishing new and strengthening the existing referral network model that facilitates linkages between different service delivery points to create an orchestra of interlinked community to facility health care service delivery in the project area

f)Track and ensure at least 95% referral completion among project beneficiaries

g) Strengthen client appointment and tracking systems at the supported health facilities to actively identify missed appointments and institute timely client follow up mechanisms for enhanced continuum of care.

h) Document best practices at community level and within the referral and linkage network in form of case studies, success stories and share at various forums at district level, project and national level.

  1. Data Clerk- 1 position
Job Title: Data clerk
Job Location: Kasese
 Job Summary:

The Data clerk shall be the custodian of all Community Linkage & Referral data as well as OVC data. He/she will ensure that all data tools used in the Facility-Community Framework and OVC are duly filled; Conduct periodic data quality checks in OVC case files, OVC Registers, and Community-Facility Referral Register among others; Organize and facilitate monthly performance and data reviews at supported health facilities; Compile and submit periodic reports.

 Required Qualification, Experience and Competencies

Essential:

a.       Diploma in Medical Records Management, Library and Information Science, Project Planning & Management, Information Technology or any related qualification.

b.      At least 2 years’ work experience in data/records management particularly related to HIVAIDS or OVC.

c.       He/ she must have demonstrable knowledge in records management, storage and retrieval

d.      Should be able to conduct data analysis and generate a report to inform programming

e.       Good communication and interpersonal skills.

f.       Demonstrable good communication skills in both English and Local Language

g.      Proficiency in various MS office packages and working knowledge of English and local language(s) are crucial.

h.      Hard working, trust-worthy, and holding the highest levels of integrity and confidentiality.

i.        Must be flexible, reliable, and able to work under pressure.

 Desirable:

j.        Additional training in M& E shall be an added advantage.

Key Duties and Responsibilities

  • Ensure regular and timely  data entry into web based and/ or any other  project data base
  • Assist   Management to develop and maintain/implement the project monitoring and Evaluation plan
  • Develop and maintain appropriate project data and information internal management systems for the project field office
  • Ensure compliance to data quality standards during collection, entry, and analysis of all project data
  • Ensure regular provision of required data to the management.
  • Ensure collection of data on all project indicators in assigned areas
  • Support the preparation of periodic or other project reports

  1. Social Worker – 1 position
Job Title: Social Worker
Job Location: Kasese
 Job Summary: The Social Worker will be responsible for overall supervision, Coaching and Mentoring of Assistant Social Workers and Para-social Workers for effective OVC household’s identification / assessment and enrollment into the Baylor Uganda OVC Program in line with PEPFAR OVC priorities, and helping OVC Households to develop Household Improvement Plans (HIPs).

 Required Qualification, Experience and Competencies

Essential:

a.       The ideal candidate for the Social Worker job placement should preferably hold a Bachelor’s degree in Social Work & Social Administration, Development Studies, Community Psychology, Guidance and Counselling, or any other related field.

b.      A minimum of two years of exposure and experience in community/ social work particularly on OVC and/ or HIVAIDS donor-funded projects

c.       S/he should have demonstrated ability to manage community volunteers and experience in working with rural communities

d.      Ability to multitask and work under pressure to deliver results

e.       Good computer skills including MS Word, Excel, PowerPoint, data entry etc.

f.       Excellent communication skills in both English and Local Language

g.      Good understanding of PEPFAR OVC Priorities

 Desirable:

h.      Additional training in Project Monitoring and Evaluation or Project Planning and Management shall be an added advantage.

i.        Ability to ride a motorcycle with a valid Class-A Driving permit

a.       Work jointly with the District Community Development Office and the District Probation Office to identify suitably competent Para-Social Workers to engage in OVC service provision.

b.      The Social Worker will ensure supervision and maintenance of an effective referrals & linkages system, as a core part in Case Management processes, thereby ensure proper documentation of beneficiaries reached to access cores services.

c.       The Social Worker will supervise Para-social Workers to effectively perform assigned tasks in respective Sub-counties such as: home visits, referrals, case follow-ups and ensure completion of referrals.

d.      The Social Worker will work in liaison with the Community Development Officer (CDO), DPC and other relevant stakeholders to organize Sub-county-level Integrated Case Conferencing Sessions, as well as mobilizing coordinating monthly meetings with Para-social Workers;

e.       Preparing and submitting timely and informative project periodic activity reports.

Additional Duties

Job title: PHE Act Officer

Report to: Executive Director

Role definition: Implement and monitor integrated Population, Health and Environment Interventions.

Key Result Areas

  1. Support training of village health teams in family planning (FP), maternal, newborn, and child health (MNCH), adolescent and youth sexual and reproductive health (AYSRH), HIV and Malaria prevention, and engage men on FP/maternal child health (MCH) and HIV prevention (and increasing risks based on declining fish stocks- “fish for sex”), building on the community’s acceptance and comprehension of the population, health, and environment (PHE) framework and its interlocking priorities.
  2. Strengthen supervision and referral links for reproductive, maternal, newborn and child health (including supervision/monitoring of VHTs/CHWs)
  3. Provide appropriate training to health care providers on (FP/HIV, syndromic management, PMTCT, CT, YFS, and Malaria) as well as referral
  4. Conduct daily integrated PHE sessions, with model households, about the consistent use of ITNs, malaria prevention, adherence and indoor residual spray.
  5. Support periodic continuous medical education for HSP with more focus on HIV and Malaria prevention treatment and care.
  6. Provide technical support for sub-county, district and health facility staff to do supervision and monitoring of CHW/VHTs.
  7. Strengthen linkages with higher level health facilities for routine viral load monitoring and timely interventions
  8. Train health care providers on FP/HIV, syndromic management, prevention of mother-to-child transmission, counseling and testing, youth friendly services, Malaria and referrals.
  9. Train and work with model households’ and different community groups to support people (especially mothers and children under age five) tested positive and initiated on ARVs to ensure adherence.
  10. Conduct outreaches for counselling and testing, including to HIV+ women and their children on issues related to nutrition, breastfeeding, WASH, FP, and immunization
    Strengthen referral links through capacity building of community groups on tracking referrals and conducting follow ups of the referred clients. The project will initiate and support periodic meetings between community agents and health workers. This will also include revision of the referral tools the project has been using to capture Malaria and HIV
  11. Support health facilities and community linkages and collaboration to scale up integrated community case management for malaria
  12. Train model households in energy saving stoves construction
  13. Establish and support existing model households that practice positive PHE behaviors across health, water, sanitation and hygiene and environment sectors
  14. Train model households on conservation agriculture and support them with improved inputs and agroforestry seedlings.
  15. Provide agriculture inputs for nutritious foods to homes with children and women affected and infected by HIV
  16. Establish and/or strengthen school PHE clubs (training of PHE club members and patrons, school PHE exchange program, IEC materials etc.)
  17. Hold dissemination events to share PHE project strategies, experiences and best practice in Ntoroko and Kamwenge District
  18. Hold regular meetings with the government officials and stakeholders to share information and strengthen collaboration for improved coordination and fundraising.

Job specifications

The job holder should have; 1) A degree in any Social Work (Agriculture, community development, business studies and commerce etc) from a recognized University. (2) Have Key personal competencies in Team and communication skills, networking and partnership skills. Must be dynamic, Persuasive & convincing, self- motivated and able to demonstrate high initiative. This is an entry level position so even fresh graduates can apply.
