Job title: OVC Social Worker

Report to: Executive Director

Deadline: October 18, 2017

Role definition: Implement and Monitor OVC Services in supported sub-counties.

 Key Result Areas

  1. Monitor and implement the OVC work-plan of funds disbursed on a quarterly basis
  2. Liase with Health facilities and Community development officers to identify, assess and enroll OVC infected and affected by HIV/AIDS
  3. Maintain a competent & motivated “team of para-social workers” by ensuring capacity building and timely payments of the monthly stipend
  4. Conduct monthly sub-county meetings for para-social workers and Community Development officers; share lessons learnt, achievements and discuss challenges and generate practical solutions
  5. Monitor case Management activities of the para social workers and routinely update the household case files at located at the CBO.
  6. Monitor distribution, storage and usage of OVC data tools and ensure timely updating of the data base and timely submission of OVC MIS reports on a quarterly basis
  7. Carry out referrals and linkages for child protection & legal support and provide a service to the referred OVC
  8. Monitor the implementation of linkage and referral process for OVCs from Health facilities to communities and support OVCs to receive services referred for and document all referrals
  9. Monitor the functionality of formed VSLA groups and monitor apprenticeship training by young people on the program
  10. Prepare monthly and quarterly reports on OVC implementation and document monthly reports and capture success stories

Job specifications

The job holder should have; 1) A diploma in any Social Work related course (2) A degree in any Social Work related course is an added advantage (3) have Key personal competences in Team and communication skills, networking and partnership skills. Must be dynamic, Persuasive & convincing, self- motivated and able to demonstrate high initiative.

Brief Description
Rwenzori Center for Research and Advocacy (RCRA) in partnership with Baylor-Uganda, is
implementing a programme to strengthen linkages from the community to the health facility and
vice-versa. Responsible to provide a range of social services and basic clinical services to
referred clients, conduct referrals and linkages to the facilities, update the registers, conduct
OVC assessments, identification and enrollments as well as report to the Districts.
RCRA therefore invites applications for the following job positions to join the project
implementation team a Linkages and Referral Supervisor (1), Linkages and Referral Assistant (7)
and Community Health Workers (7). All interested individuals should submit their application
and academic certificates to the sub county HC III or CDO’s Office , Baylor-Uganda cluster
Office in Kasese or at RCRA Kasese Office located opposite URA.
Only applicants from Hiima, Bugoye, Kitswamba, Kyabarungira, and Maliba Sub County

1. Position Title: Linkage and Referral Supervisor (1)
Reports to: Executive Director
Day to Day: According to the organisation’s Organogram
Technically: Community Linkage officer, Baylor Uganda / Social Worker, RCRA
Role definition: Ensure bidirectional linkages between health facilities and communities while
strengthening implementing activities along the 90-90-90 targets
Key result areas;
1. Liase with health facilities to identify and recruit eligible Linkage and referral Assistants as well as
Community Health workers to provide linkage and referral activities for Health facility/Community
Based services
2. Monitor the implementation of linkage and referral process for OVCs and adults from HF to
community as well as from community to HF
3. Conduct joint monthly site level meetings between HFs and community Health Workers and
Linkage and Referral
4. Monitor the functionality of network groups for PLHIV, psychosocial support groups VHTs
in order to improve quality of care and strengthen Retention
5. Identify eligible OVCs through Health facilities, provide services and track the utilization of
these services in line OVC program objectives
6. Liase with the Health facilities to ensure Family tracking forms are being filled out to ensure index
contact tracing
7. Receive and review assessment and follow up forms on a weekly basis-every Friday from the
Community Health workers and update the Community Database based at the Community Based
organisation
8. Ensure proper storage and usage of OVC and community data tools/registers, time reporting,
update of psychosocial related data in the HMIS or MIS for MoGLSD by the
community/districts/ health facilities

Qualifications & job specifications:
 Minimum qualification of a diploma in any Humanities discipline
 Additional training in Monitoring and Evaluation will be an added advantage
 At least two years’ experience in Community development programs
 Staff will allocate 100% level of effort to the project
2. Position Title: Community Health Worker (7)
Reports to:
Technically: ART In-Charge
Day to day: Linkage and Referral Assistant
Role definition: Lay health workers in the community tasked to identify PLHIV/ TB clients, other
priority populations and ensure referral and successful linkage to Health facilities to access HIV services
and other services including OVC.
Key result areas;
1. Conduct initial household assessments for all mapped clients within the catchment area and
subsequently conduct quarterly visits once each quarter to enforce adherence, support disclosure and stigma reduction
2. Identify households of index clients not tested and refer to health facilities for targeted home-based HIV Counselling and Testing and TB screening
3. Work with health workers conducting targeted index client testing at household level community and
promote same day linkage by writing referrals of newly diagnosed PLHIV and support with physical referrals for initiation into HIV care
4. Using the HTS screening tools at household level mobilise men, adolescent girls and OVCs and refer for testing services
5. Conduct client tracking and follow up of clients who do not keep appointment including loss to follow u and undocumented transfer outs to ensure successful referrals
6. Educate PLHIV on importance of PLHIV groups and refer them for ongoing community care and support for retention in clinical care
7. Conduct follow up activities for exposed infants and eMTCT mothers to ensure completion of the 18 months cascade as well as adolescents
8. Assess HIV or TB infected children for OVC service provision and link to other services in the
community according to identified need

3. Position Title: Linkage and Referral Assistant (7)
Reports to: ART In-Charge
Technically: Linkage and Referral Supervisor at RCRA
Duty Station: Health Facility
Role definition: Main role is conduct linkage and referral activities for PLHIV and TB including
PMTCT mothers, exposed infants, general clients on ART, SGVB survivors as well as OVC infected
and affected by HIV and AIDS.
Key result areas;
1. Identify PLHIV that need community care and support from health facilities where they are attached
and refer those to Community Health Workers nearer to their homes for ongoing psychosocial
support
2. Ensure referral and client locator maps are available and regularly updated both at the ART clinic
and the Mother Baby Care Point to ensure completed referrals
3. Consenting of all PLHIV at health facilities for home visits and Home Based HIV testing
4. Liase with the RCT volunteers to receive a Family tracking form for index client testing
5. Have an updated registry of all PLHIV support groups in the catchment area of the Health facility
to support continued community care
6. Provide support to escort/accompany clients at risk of not completing referrals to health facilities and
other services within 3 days and must ensure that the client receive the service
7. Document all referrals in and out referrals follow up to obtain feedback and update the community
health facility linkage and referral register on a daily basis for all referrals received from the
community
8. Ensure patient appointment books are updated each clinic day and ensure that all missed
appointments are linked to Community Health workers for Real time follow up
9. Liase with the ART In-charge to send out bulk reminder messages one week before the scheduled
appointments for clients in care
10. Fill out the monthly client tracking, linkage and retention tracking tools on a weekly basis
11. Provide monthly reports on linkage and referral activities and submit to RCRA and ART clinic In-
Charge
